Out Of State Medical Marijuana Card Visiting Patient Form

If you're visiting from another state and have one of the qualifying medical conditions you may apply to the Arkansas medical marijuana program as a visiting patient.

You will need your home state medical marijuana card or equivalent and apply for an out of state registration.

Your visiting patient application will cost $50 (non-refundable). If approved you will be able to purchase medical marijuana in Arkansas for a 30-day period, per each application you submit.

Currently all out of state registrations must be complete online via the AMMSys Portal.

Physician Written Certification Form Arkansas

The physician written certification form is required to apply for the Arkansas medical marijuana program.

Doctors of medicine or Osteopathy licensed in the state of Arkansas, have a controlled substances license on file with the DEA, be in good standing to practice medicine in Arkansas, and have a bona fide physician-patient relationship with the patient they are certifying for medical cannabis can provide you with a written certification.

Currently the Arkansas Department of Health (ADH) does not maintain a list of qualified providers.

Remember your physician certification is valid for 30 days, if you fail to submit it to the ADH whiting 30 days, you will need to get a new certification.

Qualified registered patients are allowed to purchase/possess up to 2.5 ounces or 71 grams of medical dispensary from a dispensary every 14 days.

How long does it take to get your medical card in the mail in Arkansas?

According to the Arkansas Department of Health it currently takes 10 to 14 days to process applications. Adding the time it takes to mail, you should receive your medical card in the mail within 30 days upon being approved.

  • Physician Written Certification
  • Patient Application
  • Copy of your Arkansas driver's license or ID
  • Pay the $50 processing fee
